Request Service (Moves, Adds, Changes)

Please click on the Service Request Form link below to request cabling, activation or deactivation of network drops, DNS entries, or IP Address assignment. The link will take you to the Work Order Ticketing system; once you log in you can select the type of service desired. The reference pages provide additional information about important deadlines for requesting service.

NOTE - Using this form for reporting problems will delay response. Use the "Report a Network Problem" link instead!
