The University Undergraduate Curriculum Committee's UUCC Curriculum Approval Request Form requires that a Technology Services Resource Needs Form be completed and attached to your request. Download the form below, complete, and fax to Technology Services, (804) 828-2088. The form will be reviewed, signed, and returned by Fax to you. Although you should allow for a 2 week turnaround, the form is usually processed and returned to you within 3 days.
