VCUCC Forms

These forms allow you to request VCUCC services. Many forms require you to log in to the University LDAP directory server using your username and password in order to establish your identity.

Request to install/configure new hardware in VCUCC
You can use this form to request assistance with design, planning, and procurement of your new server system(s), and also discuss your plans for hosting separately acquired systems at VCUCC's computer facility. 

Establish a Service Level Agreement with VCUCC
Sample form in Word format. VCUCC's practice when establishing relationships is to formulate a Service Level Agreement (SLA) with customers, similar to this sample document, to clarify and emphasize both parties roles and responsibilities. Some groups with current SLAs with VCUCC are the Biotech Research Park, Client Services, the state Department of Mining, Minerals, and Energy, Facilities Management, the VCU Foundation, VCUCard, and VCUNet.

Configure a server for backup/archive using VCUCC's TSM server
You can utilize this form to request backup and archive services utilizing VCUCC's tape library systems

Request to add a new SQL Server database
You can use this form to request a new SQL Server database be added on the shared SQL Server. The database will first be added to the development SQL Server, DSS3. When development is complete, the database will be migrated to the production SQL Server.

Request eID Application ID or Guest Accounts
You can use this form to request an Application ID or temporary Guest Accounts in the University eDirectory.  This form is for use by VCU faculty and staff only.

Request New LDAP Attribute
You can use this form to request a new LDAP attribute.  This form is for use by VCU faculty and staff only.
